Your first visit to our office establishes a vital foundation for our relationship with you. During this visit, we will obtain important background information, like your medical history, and give you time to get to know Dr. Luck.

What to Expect at Your First Visit

On your first visit, Dr. Luck will review your medical and dental histories and evaluate your overall oral health and present dental condition. He will listen carefully to any dental or health concerns you may have. Dr. Luck will suggest treatment where he feels it is necessary, but, ultimately, the decision is yours. We will take high definition pictures of your teeth and we will explain to you what conditions we find.

Insurance & Benefits

Our office is “out of network” for all PPO insurance plans. What this means for you is that you will be responsible for the difference between our fee and what your insurance company will reimburse for your services. As a courtesy, prior to any treatment, we can request a pre-estimate from your insurance company so that you will know what your out-of-pocket cost will be.

Patient Forms

You can save time and expedite your first visit by completing our New Patient forms online. Also, remember to bring your insurance card and an ID with you to your first appointment. There is also an option to download the forms, fill out at home and hand them to our staff when you arrive for your initial visit to our office.
